Doc: Refine structure and placeholders for breaking changes

Follow up to #16603

• Look at folding 6.3 bc into 6.0 topic
• Experiment with wording to imply that a topic applies to all of a major release. (Currently, I'm using "8.0 series.")
• How far back do we want to keep breaking changes?
